I hereby confirm that I am informed that the personal data provided about me will be used to carry out measures at my request before concluding the contract (to give me the opportunity to familiarize myself with the school management system, the teaching form, as well as to prepare the contract). I agree to participate in surveys aimed at improving the quality of services provided by R1TV and to the collection and transfer of my provided personal data to the service provider appointed by the Controller for conducting the survey.
